GH Peptide Blends: Sermorelin, Tesamorelin and Ipamorelin

What sermorelin/ipamorelin and tesamorelin/CJC-1295/ipamorelin blends contain, how to calculate each component, molar ratios and how blends are tested.

Growth hormone peptide blends combine a GHRH-receptor agonist with a ghrelin-receptor agonist in one vial. The two most common are sermorelin + ipamorelin and the three-way tesamorelin + CJC-1295 + ipamorelin. For laboratory work the issues are the same as for any blend: knowing the exact amounts, calculating each component separately, and verifying all of them.

Key points

  • Blends pair compounds acting on two different receptors that converge on GH release.
  • Equal weights do not mean equal molar amounts; ipamorelin is far lighter than the GHRH analogues.
  • Calculate concentration per component, never as a single total.
  • Component masses differ enough (712 / 3368 / 5136 Da) for straightforward MS verification.
  • The combinations themselves have no peer-reviewed trial data that we found.

Common blend compositions

BlendTypical contentReceptors engaged
Sermorelin + ipamorelin5 mg + 5 mg (10 mg total)GHRH-R, GHSR
CJC-1295 (no DAC) + ipamorelin5 mg + 5 mg (10 mg total)GHRH-R, GHSR
Tesamorelin + CJC-1295 + ipamorelinOften 2 + 5 + 5 mg (12 mg total)GHRH-R (×2), GHSR

Ratios vary between suppliers, and a “12 mg blend” tells you only the total. The certificate of analysis should state each component’s amount. Individual products: sermorelin acetate, ipamorelin, tesamorelin.

The rationale, and its limits

GHRH-receptor and ghrelin-receptor signalling are distinct. GHSR has constitutive activity that ghrelin raises and LEAP2 lowers (Cornejo et al., 2021), whereas the GHRH receptor is a conventional cAMP-coupled GPCR. Antagonist experiments in the original ipamorelin study confirmed that ipamorelin works through a GHRP-type receptor rather than the GHRH receptor (Raun et al., 1998). Engaging both is therefore a reasonable experimental design.

What the rationale does not establish is that a particular ratio is correct, or that the combination outperforms either component. Both of those would need comparison studies that, as far as we found, have not been published for these blends.

The molar ratio problem

Blends are sold by weight, but receptors respond to numbers of molecules. Because the components differ greatly in mass, equal weights give very unequal molar amounts.

ComponentMassMolecules per 5 mg (relative)
Ipamorelin711.9 g/mol1.00 (reference)
CJC-1295 (no DAC)~3368 g/mol0.21
Tesamorelin~5136 g/mol0.14

In a 5 + 5 mg sermorelin/ipamorelin vial, there are roughly 4.7 ipamorelin molecules for every sermorelin molecule. Any dose–response reasoning should use molar amounts, not milligrams.

Reconstitution maths

VialSolventComponent concentrations
10 mg (5 + 5)2.0 mL2.5 mg/mL each
10 mg (5 + 5)1.0 mL5 mg/mL each
12 mg (2 + 5 + 5)2.0 mL1.0 / 2.5 / 2.5 mg/mL
12 mg (2 + 5 + 5)3.0 mL0.67 / 1.67 / 1.67 mg/mL

Use the peptide reconstitution calculator one component at a time; the general procedure is in how to reconstitute peptides.

Verifying a blend

Three checks matter:

  • Presence of every component. MS should show all expected masses. A missing peak means a missing ingredient.
  • Ratio. Calibrated HPLC peak areas against standards. In a 2 + 5 + 5 blend, tesamorelin is only one sixth of the content, so the method must be sensitive enough for the smallest component.
  • Purity of each. Degradation products of one component can co-elute with another; a stability-indicating method is preferable.

Validated LC-MS/MS methods exist for the whole GHRH analogue family, with characterised in vitro metabolites and detection limits generally at or below 1 ng/mL (Memdouh et al., 2021).

Storage

Store the sealed vial at −20 °C and reconstituted solution at 2–8 °C, protected from light, without repeated freeze–thaw. In a mixture the least stable component sets the useful life, and here that is likely to be the GHRH analogue: sermorelin and tesamorelin both contain an oxidation-prone Met27, while ipamorelin has neither methionine nor cysteine. Unbuffered water offers none of the pH control that formulation reviews identify as the main practical stabiliser (Nugrahadi et al., 2023), so date every vial. See storing reconstituted peptides.

Open questions

  • Is the GH response to a blend additive, synergistic, or no greater than the stronger component alone?
  • Does the GHRH analogue degrade faster when stored alongside a secretagogue?
  • What molar ratio, rather than weight ratio, would make mechanistic sense?

Frequently asked questions

What is a sermorelin ipamorelin blend?

A single vial containing both peptides: sermorelin, a GHRH(1-29) analogue acting on the GHRH receptor, and ipamorelin, a pentapeptide acting on the ghrelin receptor. The two engage different receptors that both lead to growth hormone release.

What is in a tesamorelin CJC-1295 ipamorelin 12 mg blend?

Three peptides in one vial, commonly 2 mg tesamorelin, 5 mg CJC-1295 (no DAC) and 5 mg ipamorelin, though ratios vary by supplier. Always check the certificate of analysis for the exact amounts.

How do I reconstitute a three-peptide blend?

Exactly as for a single peptide, but calculate each component separately. With 12 mg total in 2 mL, a 2/5/5 mg vial gives 1, 2.5 and 2.5 mg/mL respectively.

Do blends have published research behind them?

The individual peptides do. We are not aware of peer-reviewed trials of these specific combinations, so the rationale comes from the separate receptor literature.

Can the components be told apart analytically?

Yes, easily. Ipamorelin is about 712 Da, CJC-1295 about 3368 Da and tesamorelin about 5136 Da, so mass spectrometry and HPLC resolve all three.
